Bernstein frames the roughly $11 billion global contact lens market as a defensive compounder growing 4–5% a year, propelled by rising myopia prevalence and premiumization toward daily disposables and specialty lenses (toric, multifocal). The industry is a tight oligopoly: Johnson & Johnson, Alcon, CooperCompanies and Bausch + Lomb control nearly 90% of global revenue. Despite pricing pressure—especially outside the US, which accounts for 57% of the market—high entry barriers, optometrist brand loyalty and patient stickiness support stable economics. Daily disposables already represent 62% of retail value, while roughly 80% of demand comes from repeat purchases by existing wearers, leaving only a small share contestable each year. Alcon, ranked second globally with about 24% share, is expected to keep gaining share on its new product cycle and capacity investment (DSM Flex), helped by strategic uncertainty at Cooper. Growth proved macro-resilient during the global financial crisis, slowing only from high single digits to low single digits. Bausch + Lomb's next-generation bioactive lens, Project HALO, is expected in 2028. Target prices: Alcon CHF 76.15 (USD 92.35 for ALC US), Johnson & Johnson USD 261, Hoya JPY 35,500, EssilorLuxottica EUR 185, Carl Zeiss Meditec EUR 26.10.
